The process was confusing so we asked for help to arrange respite activities that would provide supervision and recreation that matched their child’s developmental stage, including community outings (e.g. school, appointments or a programs).”

“Before I found out about Holistic’s Fast Track Program, I didn’t realize that HCBS offers many support services that improve a caregiver/family’s ability to care for a child with special needs in their home or community. I didn’t understand the application or how to find the information that the Care Manager needed.”
Holistic explained that getting a care manager was the first step. Line by line, they showed me how to fill out the application so we could find out if we were eligible for HCBS caregiver support. Once the approval came through, the Navigator arranged a meeting with a mental health provider who helps me advocate for my child’s needs both at school and at the doctor’s.”

School psychologist Tammy W. had hit a wall with the HCBS process for obtaining prevocational services in which several young men at her high school had expressed interest.

“I’m working with several young teen boys who need prevocational services to prepare them to work or volunteer in the community. Some don’t yet know their own interests or strengths and need help with exploring the possibilities. We needed the right Direct Service Provider for these youngsters and couldn’t find candidates that were appropriate.”
The first thing my Holistic Navigator did was show me how to get a care manager assigned to my job seekers and then how to complete the exact paperwork needed for prevocational services. Once we got the ok for services, he connected us with several programs that were a good fit. The kids are really doing well!”
