CF Support improves a caregiver or family’s ability to care for the child in their home or community

I didn’t understand the application or how to find the information that the Care Manager needed.”
Holistic supported Gaelle with the process of getting a care manager who could determine whether her family was eligible for HCBS services. A Holistic Navigator set up a meeting with a mental health provider and continues to advocate for Gaelle and her son on their journey.
